This seems to have been caused by installing the WPML modules at the network level but only configuring one site.

It is not recommended that you network activate the plugin for different sites. Once I removed the network activation and only had the plugins activated per site, then the problem went away.

This is because the system was confused as to which URL format to apply. (see image).

I hope this resolves your issue now.
