The client was experiencing an issue with the WP External Links plugin (v2.60) that was preventing the translation tools tab from loading on his website.
Solution:

Problem:
Language flag icons not showing due to LiteSpeed Cache plugin caching.
Solution:
Excluded the flag images from caching (added image css classes to the Media exclude section of LiteSpeed Cache plugin).
