I checked it in German, and it only outputs “5.” This suggests that the shortcode responsible for loading it might be broken.
Could you please confirm whether the form in the original language was loaded via a shortcode, or if you had a selection field to choose the specific form ID?

You can try setting the ACF Field Type to “Copy” (in the Field Groups setting), so the post ID will be duplicated for the translation and automatically updated to the translated post ID.
Thank you for your feedback. I tried changing the number, but the problem persists. It seems to me that this is more likely due to the fact that the form isn't selected by default, even though it is in French, but not for other languages, and it's impossible to do it manually for product types.
I manually integrated the forms for all EN and GER languages, and I'll work on the Spanish version this week. However, it's impossible to manually integrate the form in all categories, like this one: hidden link

thanks for the info! It looks like the shortcode structure is being modified, instead of showing as [shortcode_example id="5"], it’s being reduced to just 5, which makes it unselectable.
I would eventually also need to request temporary access (WP-Admin and FTP) to your site
– preferably to a test site where the problem has been replicated if possible –
in order to be of better help and check if some configurations might need to be changed
