I understand what you're looking for. I installed the "Fixed-TOC" plugin in my development environment and understood the process of translating that text. Here's how you can do that.
- Please open the default page in WordPress Editor.
- Scroll down to the "Fixed TOC" meta
- Expand the "Contents Header" and click on the "Grey Icon" beside 'Title' (Screenshot: )
- Make a small change in the title of the page and Update the page.
- Next, go to WPML > Settings and scroll to the "Custom Field Translation"
- Click on the "Show System Fields" link
- Then search for "_fixed_toc" and set the translation preference for the field as "Translatable"
- Once done, open the default page again and make another update.
- Now, proceed to update the translation by clicking on the update icon(hidden link).
The text "Contents" should now appear in the Translation Editor and you can add translations for the text as per your needs. This is how(screenshot: hidden link) the text appears on the translated page.

I understand your point. However, because the text is placed in a dynamic custom field, it will only be displayed as translated once it has been modified through the WordPress editor and translated using the translation editor as I shared in my last response.
FYI, After you input a translation, it gets stored in the translation memory. Consequently, this translation will automatically load when you're translating a page or post where the Fixed TOC is activated and the 'Contents Header' has been modified. If automatic translation is enabled, this specific text will be translated at no extra cost.
