The first question here on NL form is translated
But the second and third questions are not
(I'm aware the account has run out of pre-paid translation credits, and have requested the company add more credit, however this form was created a long time ago and almost certainly before the credits ran out)

When sending it to translation, make sure to check "Translate myself", this way you can make sure you'll be looking at the translation as it is right now 🙂
