Hi,

I see that the form is created with the Gravity Forms plugin, please make sure you have the Gravity Forms Multilingual plugin installed (https://wpml.org/download/gravityforms-multilingual/?section=changelog) then follow this guide to translate the forms: https://wpml.org/documentation/related-projects/gravity-forms-multilingual/#translating-gravity-forms
