This thread is resolved. Here is a description of the problem and solution.

Problem:
The client is trying to display a translated version of a form for Spanish users.
Solution:
We recommend following these steps:
1. Make a small change to the page content where the form is embedded.
2. Save the changes.
3. Update the translation of the page.
After these steps, the translated form should display correctly on the translated page.
If you're experiencing a similar issue, we suggest trying these steps. However, please note that this solution might be outdated or not applicable to your case. If the problem persists, we encourage you to open a new support ticket. We also highly recommend checking the related known issues, verifying the version of the permanent fix, and confirming that you have installed the latest versions of themes and plugins.

Is there any documentation that you are following?

Hi there,
Thanks for contacting WPML support.

Please try the following steps:

1. Edit the original form
2. Update it with a minor change (You can undo the changes later)
3. Send again for translation

If that didn't help, kindly create a full backup of the database and website then go to WPML > Support > Troubleshooting page, and click the following:

- Clear the Cache in WPML
- Remove ghost entries from the translation tables
- Fix element_type collation
- Fix WPML table collation
- Set language information
- Fix post type assignment for translations

Thanks for the tip. Unfortunately, I tried both things and is still missing the form in the Spanish version. What should we do next?

I would need to request temporary access (WP-Admin) to investigate the issue further.
Preferably to a test site where the problem has been replicated.

Hi Yitzhak,

I will take care of your ticket from now. On your staging site, I just make a small change to the page content > Save > Update the translation and the form displays on the translation page as well.
