The redirect will work the first time the user visits your site. WPML Redirect Based on IP Country has a 120min session expiration, so it will happen either the first time somebody has visited OR after the session expires.
This is done to allow users to change their language, otherwise, there would be a constant redirect back to German creating a loop.

Regarding the co.uk question, please provide me some additional info to better understand.
If I understad correct you want to use co.uk instead of .com
At the moment the EN version is .com/en since you are using the directories language URL format. To use .co.uk it would require you to switch to "a different domain per language"
