I do not see an issue here, as it is expected to have one hreflang for each active language plus one for x-default. See screenshot. I am sadly not able to see your Spanish website due to a location block. Could you please clarify what you are expecting to see? Best regards Andreas

It seems like the permalink is not building up correctly. To understand the issue I will need to take a look into the backend. I would like to request temporary access (wp-admin and FTP) to the site to investigate the issue further. The required fields are below the comment section when you log in to leave the next reply. The information you provide is private, meaning only you and I can see and access it. IMPORTANT Please be sure to back up the site and database before granting us access. I would not suggest building a slug this way. Go to hidden link and change the slug to: hand-wrist-and-forearm-injuries The translated Spanish page appears here: hidden link This is expected, as by default WPML will use the title as slug. --- Also, take note that the Spanish page is not yet published to Headless Hostman: "This page has not been published to your live website. Push the red button to get it there, when you are ready."
